Faith Cutrona F.N.P was born and raised in the Detroit Metropolitan area. She Graduated from Southfield- Lathrup High school and moved to Glendale, AZ to join the rest of her family in 1982. She is married to her loving husband of 28 years Jon Cutrona. Their marriage has given them 3 boys ages 15, 20, 22. She enjoys taking cruises when time allows and whenever spare time is available she enjoys reading and shopping.
Faith attended Phoenix College and acquired an associate’s degree in Nursing and started working at Del Webb Hospital after graduating in 1988. She returned to school in 2004 and finished her BSN while still working for Del Webb. Her career at Del Webb ended in 2005, but she continued nursing at Banner Estrella hospital. She spent 20 years working in the emergency department as both Staff Nurse and in the Management role.
While working for Banner Estrella E.R she was attending the University of Phoenix to obtain her MSN, FNP Degree. With strong will and determination she successfully completed her goal and became a Board Certified Family Nurse Practitioner in 2008.
As a Certified Family Nurse Practitioner she has worked in both Family Practice and Urgent Care. She currently provides the highest quality care for her patients at Choice Medical Walk-In. Here she enjoys the opportunity to educate patients on wellness/preventative care, chronic and acute medical illnesses. Another facet of her professional interest includes Bio-Identical Hormone Replacement Therapy using compounded medications specific to each one’s own needs. In her own words:

Dr. Brett Willden spent most of his childhood in Arizona, graduating in the top 1% from Chaparral High School. He started his college career at UC San Diego, before taking a 2-year hiatus to serve a church mission in Argentina. When he returned, he attended BYU where he majored in Spanish with a pre-med emphasis.
He was accepted to Kirksville College of Osteopathic Medicine (KCOM), the founding school for osteopathy. Osteopathy is an approach to medicine which combines the traditional M.D. philosophy with a more holistic attitude including manipulation, nutrition, etc. Dr. Willden graduated from KCOM in 1999 in the top 10 of a class of more than 150 students. He completed his internship and residency at John C. Lincoln Health Network in Phoenix, AZ. He is board certified in Family Practice.
Dr. Willden’s professional career began in residency where he worked at John C. Lincoln Deer Valley Emergency Department and at Mendy’s Pediatric Emergency Room. He set up private practice in New River in 2002 before expanding to Anthem shortly after. In 2004, he joined Gavilan Peak Family Practice where he practiced until February 2008. In 2010 he and Dr. David Foy joined forces to establish Choice Medical Walk-In where his professional interest is focused on providing the highest quality care for his patients.
On a personal note, Dr. Willden lives in Anthem, AZ with his wife and three children. He enjoys spending time with his family, coaching sports teams, boating and ATVing. He is very involved in church activities and enjoys traveling, especially to Mexico where he can practice his Spanish. The Phoenix Suns and Arizona Diamondbacks are his favorite sports teams.
Dr. David B. Foy was born and raised in Chicago and attended Palatine High School. He started his college journey at the University of Iowa, but decided to return to Chicago to avoid paying high out of state tuition fees. He finished off his B.S in Biology at Northeastern Illinois University in Chicago. Once done with his B.S he took his MCAT test for medical school. In 1992 he was accepted to Dr. William Scholl College of Podiatric Medicine and achieved his D.P.M in 1996.
Dr. Foy enjoyed being a foot and ankle physician, but found himself wanting to treat the whole patient and all their ailments. He decided to apply for Osteopathic Medical School at Midwestern in Chicago and was accepted in 1996. He attended Midwestern in Glendale AZ and achieved his D.O degree in medicine in 2000.
He started his professional career as a physician after medical school while doing his Family Practice Residency in Knoxville Tennessee from 2000 to 2003. He became Board Certified in Family Practice that same year and returned to AZ to practice medicine for John C. Lincoln Urgent Care. He practiced at JCL Urgent Care for 5 years where he and Dr. Willden initially met.
In 2010 he and Dr. Brett Willden joined forces to establish Choice Medical Walk-In where his professional interest is now focused on providing the highest quality care for his patients.
Toni Thomas F.N.P
Toni Thomas was born and raised in Madison, Wisconsin. She moved to the great state of Arizona in 1977 when she attended Phoenix College. She finished her associate’s in nursing at Phoenix College in 1979 and continued her education at ASU obtaining a Bachelor’s Degree in Nursing by 1984.
Her career as a nurse started in 1979 at Boswell Hospital working in the Coronary Intensive Care Unit (ICU). In 1983 she became the clinical Manager for Paradise Valley’s ICU and worked there until 2010. During that time period she had been attending University of Phoenix’s Nurse Practitioner Program in order to pursue her dream to do mission work for her church. She accomplished her goal of becoming a licensed Nurse Practitioner in 2010. With 31 years of ICU experience as a nurse and now a nurse practitioner she became a part of Choice Medical Walk In’s exceptional staff in 2011. Her patients frequently praise her for her heart felt patient care and the endless compassion.
Toni takes pleasure in spending time with her daughter and participating and being as active as possible with her church. The main reason she went back to school for her FNP was to pursue her retirement dream of doing Christian Mission work in Angoon, Alaska. She eventually hopes to retire there and provide medical services to the remote Indian village around that area. In her own words
